There may be a fee based on the type of request. There is no fee for patients requesting and accessing their medical records through MyUnityPoint patient portal or to direct your records to a provider for continuing care. Postage fees apply if the request needs to be mailed. Media formats available include paper, CD, and DVD. Please bring identification with you, if you choose to pick up your medical records. Your request will be processed, and we'll either mail your copy to the address specified on the authorization form, or you may pick up your copy from our front desk at the UnityPoint Health location near you. If the patient is a minor (less than 18 years of age), a parent or legal guardian must sign the authorization and complete the parental rights notification. If these options are not convenient for you, we can arrange a way for you to pick up your records. Requests can be mailed, sent through MyUnityPoint or by email.

To request medical records from your clinic, submit a request form to your local UnityPoint Clinic location or contact the clinic directly.
